Since many faculty change their testing policies from one exam to the next, please submit this form for each examination.

Important Notes:

If you postpone, or otherwise change, an exam date, or if you change an exam from an in-class to take-home, or visa versa, please notify UC Learning Services ASAP at 1-800-868-7000.

Exams must not exceed length of class period.

Please do not schedule exams for any holidays. Even if your campus is open on any holiday, the vast majority of sites will be closed.
